NHS Continuing Healthcare
NHS continuing healthcare (NHS CHC) is free health and social care in England, arranged and funded by the NHS for adults whose needs are primarily health-related.

It can be provided at home or in a care home. Eligibility depends on the complexity, intensity and unpredictability of assessed needs, not diagnosis, decided by a multidisciplinary team using a checklist and a full assessment across domains such as mobility, nutrition and cognition. A fast-track pathway applies near the end of life. Unlike local authority care, NHS CHC cannot be topped up privately.
